Daniel Larimer baf5531238 Refactored chain library
- remove circular dependency with fee_schedule
- unitiy build db_* as database.cpp
- move protocol definitions in separate directory
- combined some objects/evaluators
- combined limit/call evaluator/objects into market_evaluator.*
